Aged Care Project - Community Partners Program
Funded by the Department of Health and Ageing
AIMS
To facilitate increased and sustained access by culturally and linguistically diverse (CALD) communities to aged care support services in the southern metropolitan area.
To promote greater awareness among CALD seniors and family members about aged care support services resulting in informed decisions.
To promote greater awareness among mainstream aged care service providers about cultural issues relating to specific cultures, enabling more culturally sensitive service provision.
STRATEGIES
Disseminate information to elderly clients and carers and sessions to specific communities on aged care services, specific CaLD communities in the South Metro Region.
Establish aged care service providers' networks in three regions - Fremantle and surrounds; Rockingham/Mandurah; and Armadale/Gosnells; with the aim of providing better access to mainstream services for CALD clients.
Provide cultural briefings to mainstream aged care service providers including care packages and residential providers. |
